State Team Nomination Forms
Please see below links to the online State Team Nomination forms. There is now a nomination form for each state team a player wishes to nominate for. Only one nomination fee of $38 is payable, you can nominate for multiple teams. Late nominations incur an additional $55 fee.
All forms are completed on line through the links below (click on the age group). Once submitted you should receive a copy of your nomination to your email. If not received please contact admin@softballqld.asn.au to confirm the nomination has been received.
